A reminder from school health services

To: 6th grade parents whose child will be entering 7th grade for the 2012-13 school year

According to State of Ohio law, all students entering 7th grade are now required to have a Tdap (tetanus, diphtheria and pertussis) vaccine prior to entry. 

As with other school immunization requirements, students who do not provide documentation of this immunization are subject to exclusion from school.  To avoid exclusion, we are sharing this information with the families of 6th graders so that they have time to get this booster before school starts in the fall.  This vaccine can be obtained at your child’s family physician, pediatrician office or the local health department for a minimal fee.

Proper documentation of this vaccine will need to be given to the 7th grade Middle School office prior to the start of the new school year.  If you have any questions or need to inquire about procedures for exemption from immunizations, please contact the school nurse.
